Description
Arcing superiorly above the gastroesophageal junction, the fornix (fundus ventriculi) forms a dome that sits left of the abdominal midline and cranial to the gastric body. From a lateral perspective, the convexity of the fundus rises toward the left hemidiaphragm, while the cardia lies more medial and slightly inferior where the esophagus enters the stomach. The greater curvature sweeps inferiorly and anteriorly from the fundus, and the lesser curvature tracks posteromedially toward the incisura angularis. Clear topography. That lateral morphology matters because the fundus is the common reservoir for intragastric gas, producing the gastric air bubble seen on upright chest or abdominal radiographs, a routine landmark when teaching normal anatomy and when screening for abnormal free subdiaphragmatic air. Distention of the fundus also informs clinical decisions during endoscopy and nasogastric tube placement, and it is the region targeted in many anti-reflux and bariatric procedures where the relationship of the fundus to the diaphragm and esophageal hiatus guides dissection and wrap or sleeve configuration. Misreading the fundal contour can lead to missed hiatal hernia or an underappreciated fundal volvulus risk in markedly distended stomachs.
