Posterior Configuration of the Fundus of the Stomach

Description

Posteriorly, the gastric fundus forms a rounded dome superior to the body of the stomach, with its convex contour lying inferior to the left hemidiaphragm and lateral to the abdominal esophagus and cardia. Along its superomedial margin, the fundus blends into the cardial region at the gastroesophageal junction, while inferiorly it continues seamlessly into the gastric body. The posterior fundal wall faces the lesser sac, and its relationship to the spleen and the left kidney is implied through the fundus’ lateral and posterior orientation. This posterior configuration matters because the fundus sits at the center of the gastric bed, where the posterior stomach contacts the diaphragm and is separated from retroperitoneal structures by the lesser sac, an arrangement that explains why posterior gastric ulcers can penetrate toward the pancreas and why posterior wall bleeding may track into the lesser sac. For upper GI surgery, the posterior fundus is a frequent target in mobilization maneuvers, where division of the short gastric vessels within the gastrosplenic ligament permits fundal mobilization during fundoplication or sleeve gastrectomy, and the close proximity to the spleen frames the common complication of splenic capsular injury. Orientation here also supports endoscopic teaching, since retroflexed views in the stomach aim to inspect the fundus and cardia for lesions that hide from a straightforward forward view.
