Description
Posteriorly, the gastric fornix (fundus ventriculi) forms a smooth domed contour superior to the cardia at the gastroesophageal junction, with the esophageal canal entering just inferior and slightly medial to the highest point of the fundus. Laterally, the curvature sweeps toward the left diaphragmatic cupola, while inferiorly the fundus transitions into the body of the stomach along the posterior gastric wall. The serosal surface is implied, so peritoneal relationships to the lesser sac and the posterior attachments of the gastrophrenic region are the spatial context. A dorsal profile of the fundus matters because it matches how the proximal stomach relates to the diaphragm and posterior mediastinum, and it clarifies why air and fluid preferentially collect in the gastric fundus depending on patient position. Fundal anatomy is also central when teaching hiatal hernia and reflux disease, since a sliding hernia alters the position of the cardia relative to the diaphragmatic hiatus and can change the configuration of the fundic dome. Surgeons reference this region during antireflux procedures such as Nissen fundoplication, where the mobilized fundus is wrapped around the distal esophagus and excessive tension can compromise the short gastric vessels.
