Description
Seen in lateral orientation, the superior (diaphragmatic) surface of the adult male liver forms a smooth, domed convexity rising beneath the right hemidiaphragm. The right lobe dominates, while the left lobe tapers anteriorly and medially toward the midline, separated on the surface only by subtle contour changes rather than a true fissure. A glistening visceral peritoneum invests most of this surface, with the bare area inferred posteriorly where hepatic tissue abuts the diaphragm around the coronary ligament. Spatial relationships matter here because the diaphragmatic surface defines the liver’s position under the costal margin and its mobility with respiration, a constant consideration during upper abdominal surgery and cross sectional imaging correlation. Appreciating the high convexity helps when teaching why subdiaphragmatic collections often track along the right subphrenic space and why hepatic dome lesions can be difficult to access percutaneously without traversing pleura or lung. It also sets the stage for discussing referred pain to the shoulder via phrenic nerve irritation when the diaphragmatic peritoneum is involved. Faculty commonly use this lateral view in gross anatomy labs to orient students before introducing the falciform ligament, coronary ligament reflections, and the transition from diaphragmatic to visceral surface anatomy.
