Description
Arising from the left upper lobe bronchus, the apicoposterior segmental bronchus (B1+2) courses superiorly and posteriorly toward the lung apex before dividing into subsegmental bronchi and bronchioles. Its lumen sits posterior to the superior pulmonary vein branches and anterior to the more posterior pleural surface, with the segment it supplies occupying the superoposterior portion of the left upper lobe. Distally, tertiary bronchial tubes branch in a fanlike pattern that matches the conical geometry of the apical and posterior parenchyma. Small airways dominate. Segmental bronchus anatomy matters most when you need to localize disease to a bronchopulmonary segment, because B1+2 defines the airway entry point for the apicoposterior segment and guides both bronchoscopy navigation and segmentectomy planning. Obstructing mucus plugs, aspirated material, or an endobronchial lesion at this level can produce segmental atelectasis with a characteristic distribution toward the apex, and the left sided B1+2 configuration is a common source of right-left confusion in trainees who expect separate B1 and B2 branches. Bronchial wall thickening and tree-in-bud opacities also get reported by segment, so the tertiary and subsegmental branching pattern is more than academic. Pulmonology and thoracic surgery teaching files use this airway map to correlate bronchoscopy findings, CT bronchial anatomy, and operative segmentation of the left upper lobe in minimally invasive approaches. Medical publishers can drop it into chapters on bronchopulmonary segments, airway nomenclature (B1+2), or radiologic pattern recognition for segmental pneumonia and collapse.
