Description
Viewed from the left, the coronary sulcus (atrioventricular or auriculoventricular groove) forms a circumferential furrow between the left atrium superiorly and the left ventricle inferiorly. Along its left lateral course, the groove relates to the left atrial appendage (left auricle) anteriorly and the obtuse margin of the left ventricle laterally. The left atrioventricular junction is the key spatial concept here. Clinically, the left segment of the atrioventricular groove is where the circumflex branch of the left coronary artery typically travels, accompanied by the coronary sinus as it sweeps posteriorly toward the right atrium. That relationship underpins how cardiologists localize lateral wall ischemia in circumflex territory and why surgeons orient quickly to the groove during mitral valve exposure and when placing sutures near the left atrium, where the coronary sinus and circumflex artery can be at risk.
