Base of the Heart, Left

Description

Seen from the left, the cardiac base is dominated by the left atrium forming the posterior surface, with the left ventricle positioned anterior and inferior as it wraps toward the obtuse (left) margin. Superiorly, the pulmonary trunk and ascending aorta arise anterior to the left atrium, while the left pulmonary veins enter the atrial wall posteriorly and laterally. The coronary (atrioventricular) sulcus separates atrial from ventricular myocardium along this aspect, and the left coronary artery system tracks toward the anterior interventricular and circumflex courses around the base. Orientation is unambiguous. This left-sided perspective matters when you need to correlate posterior atrial anatomy with the great vessels that frame the pericardial reflections, because that relationship drives both access and risk in common procedures. Left atrial enlargement, mitral valve disease, and pulmonary venous hypertension all remodel this region, and a clear mental map helps when interpreting lateral chest radiographs and CT reconstructions where the left atrium sits directly anterior to the esophagus. During atrial fibrillation ablation, the proximity of the pulmonary vein ostia to the posterior left atrial wall and adjacent structures makes spatial accuracy more than academic.
