Description
Anteriorly oriented lungs fill the thoracic cavity on either side of the mediastinum, with the right lung positioned lateral to the heart and the left lung molded around the cardiac silhouette via the cardiac notch. The apex of each lung projects superiorly toward the thoracic inlet, while the bases sit inferiorly on the diaphragmatic domes. Costal surfaces face anterolaterally against the ribs, and the anterior borders descend toward the midline, separated centrally by the sternum and anterior mediastinal soft tissues. An anterior orientation is the workhorse view for teaching lobar and fissural anatomy that underpins physical exam and imaging interpretation. The right horizontal fissure helps localize right middle lobe processes that can be missed on a frontal chest radiograph when the minor fissure is incomplete, while the oblique fissures guide the expected spread of pneumonia and the distribution of pleural effusions along dependent regions. It also frames common procedural routes, such as anterior approaches for chest tube placement that must remain superior to the rib to avoid the intercostal neurovascular bundle.
