Seen from the lateral aspect of an adult male skull, the optic nerve (cranial nerve II) courses anterior to posterior from the orbital apex toward the middle cranial fossa, converging on the optic canal. The nerve sits superior and slightly medial to the orbital floor, then enters the short bony tunnel formed by the lesser wing of the sphenoid, where it travels with the ophthalmic artery. Tight confines. Proximally, the nerve is continuous with retinal ganglion cell axons bundled into fascicles, then transitions intracranially toward the optic chiasm. A lateral perspective clarifies why the optic canal is a common choke point in compressive and traumatic optic neuropathy, including sphenoid wing fractures and post-traumatic canal edema. It also supports teaching the surgical corridor for optic nerve decompression and the anatomic rationale behind visual loss from lesions at the orbital apex, where CN II pathology can coexist with ophthalmoplegia in orbital apex syndrome.
