Description
Seen from the left lateral aspect, the interatrial septum and interventricular septum form a continuous partition between the left-sided chambers and their right-sided counterparts, with the muscular interventricular septum lying inferior and anterior to the thinner interatrial septum. Superiorly, the septal margins converge on the fibrous skeleton, where the annulus fibrosus of the mitral valve sits left and posterior to the aortic root, and the central fibrous body anchors the septa near the membranous septum. Fibrous continuity between the aortic and mitral valves (the aortomitral curtain) is apparent as a pale, rigid bridge compared with the surrounding myocardium. Orientation matters. This lateral organization is the cleanest way to teach why the membranous septum, adjacent to the aortic valve and contiguous with the central fibrous body, is the weak point implicated in many perimembranous ventricular septal defects and the target zone during surgical patch repair. The same relationships guide electrophysiology: the atrioventricular node and His bundle sit in the triangle of Koch on the right side, but their course into the interventricular septum makes septal interventions and aortic or mitral valve surgery clinically adjacent to the conduction system. Septal attachment to the fibrous skeleton also explains why annular calcification can distort septal geometry and contribute to regurgitation or outflow obstruction. Use this asset in gross anatomy and cardiothoracic surgery teaching to orient learners to septal planes, fibrous skeleton landmarks, and valve-septum continuity before moving to echocardiography or CT correlation in a left lateral projection. It also suits figure support for manuscripts on VSD classification, aortomitral continuity, and septal myectomy planning in hypertrophic obstructive cardiomyopathy.
