Description
Circumscribing the uterus is a dense vascular arcade in which the uterine arteries course medially within the broad ligament to reach the lateral uterine wall, then ascend along the uterine body and fundus while giving transverse branches toward the myometrium and endometrium. Venous channels form a companion uterine venous plexus that drains laterally toward the internal iliac veins, with prominent anastomoses around the isthmus and cervix. Along the superior margin, the tubal and ovarian contributions meet the uterine circulation near the uterotubal junction and cornua, creating a continuous outer perimeter of vessels. This perimeter view matters because it maps the territory encountered in hysterectomy, myomectomy, and uterine artery ligation, where the ureter passes inferior to the uterine artery near the cervix, a relationship that drives operative strategy and complication risk. It also clarifies why postpartum hemorrhage responds to uterotonic compression of the myometrium and, when that fails, to stepwise devascularization or uterine artery embolization directed at these lateral wall trunks and their arcuate branches. Bleeding is rarely midline.
