Description
Oriented in the medial (sagittal) plane, the vaginal wall is presented with emphasis on the tunica muscularis (muscularis vaginae) between the mucosa internally and the tunica adventitia externa externally. Smooth muscle bundles form an inner circular and outer longitudinal arrangement that continues superiorly toward the cervix of the uterus, with the vaginal lumen medial to the muscular layer and the outer surface facing the surrounding pelvic connective tissues. Superior and anterior relationships logically include the cervix and lower uterine segment, while the inferior extent approaches the vaginal vestibule and perineal tissues. Clinically, this layer is the mechanical substrate for vaginal distension during intercourse and childbirth, and it is the plane you mentally reconstruct when planning repair of obstetric lacerations or evaluating pelvic organ prolapse. Loss of muscular tone and altered collagen architecture contribute to anterior and posterior compartment defects, so appreciating the relative orientation of circular versus longitudinal fibers helps explain why cystocele or rectocele can present with different bulge patterns on exam. Surgeons also reference these layers when developing the correct tissue planes for vaginal hysterectomy, colporrhaphy, and placement of a midurethral sling, where dissection that strays too superficially risks mucosal perforation and too deep risks bladder or rectal injury.
