Description
Sagittal section anatomy of the tunica mucosa vaginae is rendered with the nonkeratinized stratified squamous epithelium lining the lumen and a conspicuously thick lamina propria immediately deep to it. The mucosal surface is thrown into transverse rugae, with the folds projecting into the vaginal canal and the connective tissue cores extending inferiorly toward the fibromuscular wall. Superiorly, the mucosa approaches the cervix and vaginal fornices, while inferiorly it continues toward the vaginal introitus, maintaining a close apposition to the underlying smooth muscle and venous plexus. Appreciating the epithelium to lamina propria relationship matters in day-to-day gynecologic pathology because the vagina has no true glands, so lubrication depends on transudation across this mucosa and on cervical secretions. Estrogen-dependent maturation of the squamous epithelium and the glycogen-rich superficial layers underpins the lactobacillus-dominant flora and the normally low vaginal pH, a point that becomes concrete when teaching atrophic vaginitis in hypoestrogenic states. Depth matters. Epithelial thinning with a relatively exposed lamina propria also explains friability, petechiae, and burning symptoms, and it alters the appearance of inflammatory and infectious processes on exam and biopsy.
