Description
Sagittal section through the vagina emphasizes the layered vaginal wall, with the lumen bordered by nonkeratinized stratified squamous epithelium and an underlying lamina propria that rises into transverse rugae. Superiorly, the vaginal canal relates to the cervix and external os of the uterus, while inferiorly it continues toward the vestibule, with the mucosa closely applied to the muscularis because a true submucosa is absent. Posteriorly the vaginal wall lies toward the rectovaginal septum and anteriorly toward the bladder base and urethra. Superficial epithelial cells face the lumen. That gland-free mucosa matters. Vaginal lubrication derives largely from cervical mucus and transudation across the vaginal epithelium, a point that helps explain exam findings in hypoestrogenic states such as lactation or menopause, where epithelial thinning and reduced glycogen contribute to dyspareunia and elevated vaginal pH. The rugal pattern also underpins distensibility during intercourse and childbirth, while the squamous lining frames where trauma, lacerations, and HPV-related intraepithelial neoplasia are assessed on speculum exam and colposcopy.
