Description
Posteriorly, the left auricle (left atrial appendage) projects from the superior aspect of the left atrium, its pectinate musculature confined to the appendage and meeting the smooth atrial wall at a clear junction. The base of the auricle lies anterolateral to the left atrial body, while its tip curves anteriorly toward the atrioventricular groove. With the vessels removed, the contours of the auricular neck and the transition from trabeculated appendage to laminar atrial myocardium read cleanly. Clinical attention often centers on this exact anatomy because the left atrial appendage is the dominant site of thrombus formation in atrial fibrillation, and device closure relies on accurate sizing of the ostium and an appreciation of the variable lobes and ridge at the junction with the atrial wall. A posterior perspective supports procedural thinking, since transseptal catheters and many intraoperative views approach the left atrium from behind, and the relationship of the appendage to the atrial body can be misunderstood when only anterior heart diagrams are used.
