Description
Posteriorly oriented superior (upper) lobes of the right and left lungs are rendered in color, with attention drawn to their apical segments rising into the root of the neck above the level of the first rib. From this vantage, the superior lobes occupy the superoposterior thorax, curving laterally beneath the ribs while tapering medially toward the hila where the main bronchi and pulmonary vessels enter. On the left, the superior lobe sweeps anteriorly toward the cardiac notch, whereas on the right it lies superior to the oblique fissure that separates it from the inferior lobe. A posterior or posterolateral understanding of the apical lung matters when you teach pleural relationships and when you plan procedures near the thoracic inlet. Apex-first pathology is not theoretical. Primary spontaneous pneumothorax commonly originates from apical subpleural blebs, and a Pancoast (superior sulcus) tumor at the lung apex can track into the thoracic inlet to involve the brachial plexus and sympathetic chain, producing shoulder and arm pain with Horner syndrome. This orientation also clarifies why posterior rib fractures and paravertebral approaches can threaten underlying lung parenchyma and pleura.
