Description
Posteriorly oriented anatomy frames the extrahepatic biliary tree as the common hepatic duct descends from the porta hepatis and joins the cystic duct to form the common bile duct (ductus choledochus). From this angle, the duct courses inferiorly and slightly to the right, lying anterior to the portal vein and typically to the right of the proper hepatic artery within the hepatoduodenal ligament before passing behind the first part of the duodenum. The distal bile duct then tracks in a groove or tunnel on the posterior surface of the pancreatic head, approaching the main pancreatic duct (ductus pancreaticus) near the ampulla of Vater. Posterior perspective matters because the distal common bile duct is most often obstructed where it is constrained by the pancreatic head and the intramural segment of the duodenum. A small stone lodged at the ampulla or distal duct can produce obstructive jaundice, and when the obstruction also blocks the pancreatic duct, acute gallstone pancreatitis follows. This view also maps the operative and endoscopic landmarks relevant to ERCP cannulation of the major papilla and to surgical exploration or repair of bile duct injury, where appreciating the duct’s relationship to the posterior duodenum and pancreas helps avoid missed distal strictures.
