Description
Posteriorly oriented anatomy frames the caval orifice (ostium venae cavae inferioris) as it enters the right atrium, with the inferior vena cava ascending on the right side of the vertebral column and approaching the atrial wall from an inferior and posterior direction. The lumen appears as a smooth-walled venous channel, and the transition into the atrial cavity is typically less trabeculated than the neighboring right atrial appendage. Medially, the interatrial septal region lies close by, while the atriocaval junction sits anterior to the esophageal course and posterior to the sternocostal surface of the heart. Key topography is easiest to appreciate from behind. This posterior presentation matters when teaching the anatomy of venous return and the landmarks used in right atrial procedures. Catheter and guidewire passage from the femoral vein into the right atrium depends on recognizing the atriocaval junction and avoiding misdirection toward the interatrial septum, where excessive contact can contribute to perforation and pericardial tamponade. It also supports discussion of congenital variants such as a prominent Eustachian valve or Chiari network, which can complicate device placement or be mistaken for thrombus on transesophageal echocardiography.
