Description
Seen from a posterior perspective, the distal trachea descends to the tracheal bifurcation where the carina forms a sharp median spur between the right and left main bronchi. The ridge projects posteriorly into the lumen at the tracheobronchial junction, with the bronchi diverging inferolaterally from the midline. Surrounding tracheal wall and cartilaginous framework are understood in relation to the posterior membranous trachea, which lies adjacent to the esophagus in situ. Posterior visualization of the carina matters because it is the endoscopic and bronchoscopic landmark used to confirm orientation at the tracheal bifurcation and to judge proximity of a tube tip to the main bronchi. Small shifts in head position or tube depth can convert a seemingly centered airway into right mainstem intubation, given the more vertical course of the right main bronchus and its relationship to the carinal spur. Carinal widening, blunting, or fixation also raises concern for subcarinal lymphadenopathy or central bronchogenic carcinoma, while excessive sensitivity at the carina helps explain why suction catheter contact can trigger cough, bronchospasm, and vagal responses.
