Description
Running vertically in the midline, the trachea is shown from a posterior perspective with the C shaped hyaline cartilage rings forming the anterior and lateral walls and opening posteriorly toward the esophagus. Bridging the free posterior ends of each cartilaginous arch, the annular ligaments (ligamenta annularia tracheae) appear as transverse fibroelastic bands between adjacent rings. The posterior membranous wall is completed by the trachealis muscle and fibroelastic tissue, positioned immediately anterior to the esophageal wall. Short segments of tracheal mucosa are implied deep to the framework. Posterior tracheal anatomy matters when you are thinking about the part of the airway that can move, bulge, or be inadvertently injured. Endotracheal intubation and bronchoscopy press and torque against the membranous wall, and weakness of the posterior membrane can contribute to excessive dynamic airway collapse and tracheobronchomalacia, while focal tears can produce pneumomediastinum. Surgeons also reference the annular ligaments and ring spacing when planning tracheal resection and end to end anastomosis, because the ligamentous intervals and trachealis tension influence mobilization and suture purchase. This is the flexible side of the trachea.
