Description
Posterior to the mediastinum, the root of the lung is organized around the hilum with the primary (main) bronchus occupying a slightly posterior and superior position relative to the pulmonary vessels. Annular tracheal cartilages and their incomplete posterior segments (cartilagines tracheales) are implied proximally as the airway transitions from trachea toward the carina and into the main bronchus. Pulmonary veins typically lie more anterior and inferior at the hilum, while the pulmonary artery crosses superior to the veins but remains anterior to the bronchus, a relationship that reads cleanly from a posterior orientation. Posterior relationships matter when you plan a posterior thoracotomy, interpret cross sectional imaging, or teach the hilar “rules” that help keep bronchi and vessels straight when the labels disappear. From behind, the bronchus stands out by its cartilaginous rings, and that visual cue becomes practical when tracing endobronchial disease, mucus plugging, or an obstructing hilar tumor that narrows the airway before it compromises adjacent vessels. This angle also supports teaching the classic RALS relationship (Right pulmonary artery Anterior to bronchus, Left pulmonary artery Superior to bronchus) by forcing the viewer to anchor orientation on the bronchus first. Pulmonology and thoracic surgery courses use this view to rehearse bronchoscopy anatomy in three dimensions and to explain why lymph nodes at the carina and along the main bronchi (station 7 and the hilar stations) sit where they do in relation to the root. Radiology educators will find it suited to correlating posterior CT or MRI slices through the hilum, where airway cartilage and vessel calibers can be compared quickly.
