Posteriorly oriented, this airway plate centers on a segmental (tertiary) bronchus in the lung hilum region, continuing distally from a lobar (secondary) bronchus and directed toward a single bronchopulmonary segment. Tracheal cartilages appear as annular rings (cartilagines tracheales) proximally, transitioning to irregular bronchial cartilaginous plates as the airway caliber narrows. The posterior wall relationship is emphasized, with the membranous component and adjacent connective tissue positioned posterior to the cartilaginous arc, a useful contrast when teaching the anterior cartilaginous dominance of the trachea. Posterior orientation matters because many airway procedures and pathologies declare themselves along the posterior membranous wall. Iatrogenic tracheobronchial tears from difficult intubation or rigid bronchoscopy often track posteriorly, and understanding where the cartilaginous support ends helps explain why smaller bronchi deform or collapse more readily during dynamic airway collapse.
