Description
Seen from the left lateral aspect of the neck and upper mediastinum, the trachea is rendered as a segmented airway supported by C shaped hyaline cartilage rings, with the annular ligaments (ligamenta anularia tracheae) spanning between adjacent cartilages. Each fibroelastic band bridges the inferior border of one tracheal cartilage to the superior border of the next, forming circumferential intervals that allow controlled separation during neck extension and swallowing. Posteriorly, the open ends of the rings imply the membranous wall adjacent to the esophagus, while anterior and lateral surfaces remain cartilaginous and more rigid. A flexible joint system, not a fused tube. Clinically, these ligamentous intervals explain why the trachea tolerates elongation yet remains patent, and they become relevant when you cut across the anterior wall for a tracheostomy or cricotracheotomy. Precise ring level matters: the usual target between the second and third tracheal rings aims to avoid the cricoid cartilage superiorly and reduce the risk of subglottic stenosis, while still providing stable cartilaginous purchase for a tube. In blunt laryngotracheal trauma, partial disruption at the annular ligaments can create step offs between rings and predispose to later cicatricial narrowing.
