Description
Between each C shaped hyaline tracheal cartilage ring, the annular ligaments (ligamenta anularia) form narrow bands of pliable fibroelastic connective tissue that bridge adjacent cartilages along the anterior and lateral tracheal wall. Posteriorly, where the cartilage is deficient, the airway contour is completed by the membranous wall and trachealis muscle rather than annular ligament. Superiorly these intercartilaginous connections approach the cricoid cartilage at the laryngotracheal junction, and inferiorly they continue toward the carina as the rings become more closely spaced. Annular ligaments matter because they permit the trachea to lengthen and recoil with cervical extension, swallowing, and changes in intrathoracic pressure while keeping the cartilaginous scaffold aligned. They also explain why injury or scarring across the intercartilaginous spaces can stiffen a segment and contribute to acquired tracheal stenosis after prolonged intubation or tracheostomy, where pressure necrosis and granulation often occur at the level of the cuff or stoma rather than through the cartilage itself. Endoscopic dilation and segmental resection planning both benefit from a clear mental model of which portions of the wall are cartilaginous, intercartilaginous, and membranous.
