Description

The maternal torso is shown in a lateral three-quarter orientation with the abdominal wall rendered transparent, exposing the gravid uterus filling much of the abdominopelvic cavity. Inside the amniotic sac, the fetus lies in cephalic presentation with the head low in the pelvic inlet, the spine directed anteriorly and the limbs flexed against the trunk; the coiled umbilical cord runs from the fetal abdomen toward the placental site on the uterine wall. Displaced maternal viscera are visible behind and above the uterus, including loops of small intestine pushed superiorly and posteriorly, the rectum posteriorly and the urinary bladder compressed anteroinferiorly. The lumbar vertebrae, sacrum and pelvic bone frame the uterus posteriorly, while one hand rests on the fundus and the other supports the lumbar region. At 28 weeks the third trimester begins, and this view illustrates the anatomical basis of common late-pregnancy findings: elevation of the diaphragm and fundal height near the level of the costal margin, bladder compression with urinary frequency, gastric and intestinal displacement with reflux, and lumbar lordosis with low back pain. Fetal lie and presentation assessed here by Leopold manoeuvres and ultrasound guide obstetric planning, since cephalic presentation is favourable for vaginal delivery. The relationship between uterine fundus, maternal spine and pelvic brim is also relevant to fetal viability counselling, growth surveillance and preterm labour assessment at this gestational age. The image suits obstetric and midwifery teaching sequences that track pregnancy week by week, antenatal class handouts and patient education leaflets explaining fetal position and maternal discomfort. It also serves textbook and journal figures on third-trimester maternal physiology, apps and website content for expectant parents, and training material for ultrasound and physiotherapy practitioners addressing posture in pregnancy. Anatomical accuracy verified by SciePro's Medical Advisory Board.
