Description
Positioned within the right lung’s bronchial tree, the right posterior segmental bronchus (ramus bronchialis segmenti posterioris) arises as a tertiary (segmental) branch from the eparterial right superior lobar bronchus. It courses posteriorly and laterally toward the posterior segment (S2) of the right upper lobe, running in close company with the segmental pulmonary artery branch while the segmental pulmonary veins occupy more intersegmental planes. Distally, the airway divides into smaller subsegmental bronchi that distribute to the posteriorly placed parenchyma adjacent to the posterior thoracic wall. Relationships are presented in anatomical terms: proximal at the lobar bronchus, distal toward the segmental and subsegmental arborization. Isolation of a single segmental bronchus matters because S2 disease and access are often discussed at the segment level, not merely by lobe. Secretions and aspirated material can preferentially track into posterior segments in supine patients, and bronchiectasis or postobstructive atelectasis may localize to the posterior segment when a mucus plug or endobronchial lesion compromises the S2 bronchus. Bronchoscopists use segmental anatomy to navigate from the right main bronchus to the upper lobe bronchus and then into the posterior segmental orifice, a common target for bronchoalveolar lavage or transbronchial biopsy when posterior upper lobe pathology is suspected. Small airway, big consequences.
