Description
Rightward from the pulmonary trunk, the right pulmonary artery runs horizontally in the mediastinum toward the hilum of the right lung, just inferior to the aortic arch and anterior to the right main bronchus in the typical RALS relationship at the root. Proximally, the pulmonary trunk lies anterior to the ascending aorta, then bifurcates into left and right pulmonary arteries near the level of the sternal angle. As it approaches the lung tissue, the right pulmonary artery sits superior to the right pulmonary veins and divides into lobar branches that track with the bronchi toward the superior, middle, and inferior lobes. Understanding the course and branching of the right pulmonary artery matters whenever you are interpreting CT pulmonary angiography or planning central pulmonary artery reconstruction, because clot burden in acute pulmonary embolism often lodges at the bifurcation and extends into the right pulmonary artery. The rightward projection also clarifies relationships that guide safe hilar dissection: surgeons can anticipate the artery’s position relative to the right main bronchus during lobectomy and during lymph node sampling around the superior pulmonary vein and bronchus intermedius. Misidentification at the hilum costs time. It also risks hemorrhage.
