Description
Oriented from the right side, the aortic arch rises superiorly from the ascending aorta, sweeps posteriorly and leftward across the superior mediastinum, then continues as the descending thoracic aorta. The curvature sits superior to the pulmonary trunk and left atrium, with the trachea and esophagus positioned posterior to the great vessel as it crosses the midline. At the arch’s inferior concavity, the ligamentum arteriosum spans toward the left pulmonary artery near the aortic isthmus. Typical brachiocephalic branching from the superior aspect is implied, with the brachiocephalic trunk coursing up and rightward toward the sternoclavicular region. A right-sided orientation matters because it clarifies the arch’s three-dimensional relationship to airway and esophagus, the same relationships that determine symptoms in aneurysmal dilation or acute aortic syndromes. When the arch enlarges, posterior mass effect can produce dysphagia from esophageal compression, and recurrent laryngeal nerve dysfunction may follow stretching around the ligamentum arteriosum and arch concavity. This perspective also supports procedural planning for arch repair or endovascular stent-grafting, where identifying the isthmus and the takeoff of the brachiocephalic vessels helps anticipate landing zones and cerebral perfusion strategy.
