Description
Arising superiorly from the ascending aorta, the aortic arch curves posteriorly and to the left, then continues inferiorly as the descending thoracic aorta along the left side of the vertebral column. From a left lateral perspective, the convex superior surface gives rise to the brachiocephalic trunk, left common carotid artery, and left subclavian artery, while the concave inferior surface forms the roof of the pulmonary trunk bifurcation region and relates to the root of the left lung. The ligamentum arteriosum spans from the undersurface of the arch to the left pulmonary artery near its origin. Clear left sided topography. Left sided visualization matters because it clarifies the arch’s relationship to the left main bronchus and pulmonary artery, the structures most often displaced in a thoracic aortic aneurysm. The same anatomy underpins hoarseness from left recurrent laryngeal nerve palsy when the nerve loops under the arch near the ligamentum arteriosum and becomes stretched or compressed (Ortner syndrome).
