Description
Rightward anterior presentation of the heart emphasizes the sternocostal (anterior) surface with the right atrium and right ventricle forming most of the visible contour, while the left ventricle contributes the left border and the apex inferiorly and to the left. Along the anterior interventricular sulcus, the anterior interventricular branch of the left coronary artery (LAD) typically descends toward the apex, accompanied by the great cardiac vein. Superiorly, the ascending aorta and pulmonary trunk sit centrally, with the superior vena cava entering the right atrium on the subject’s right side and the right auricle projecting anterosuperiorly. Clinically, this oblique anterior-right profile is the one surgeons and imagers think in during median sternotomy and routine echocardiographic and CT orientation, because it aligns the viewer with the right ventricular free wall and the course of the LAD on the front of the heart. That matters for anterior wall myocardial infarction, where LAD occlusion endangers the anterior left ventricle and interventricular septum, and for pericardiocentesis planning, where the right ventricle lies immediately deep to the anterior chest wall. Coronary surface anatomy becomes the map.
