Description
Arising from the right inferior lobar (secondary) bronchus, the basal segmental bronchus courses inferiorly and laterally into the lower lobe, where it divides into tertiary (segmental) branches directed toward the basal bronchopulmonary segments. Segmental bronchi run alongside segmental branches of the pulmonary artery, while the pulmonary veins track intersegmentally, marking the planes between adjacent segments. Distally, the conducting airway narrows toward subsegmental bronchi and bronchioles within the basal parenchyma. Clear segmental boundaries. Segmental airway branching in the right lower lobe matters because bronchopulmonary segments function as surgical and radiologic units, and the basal segments are frequent sites for dependent atelectasis and aspiration-related consolidation in supine patients.
