Description
Arcuate branches of the uterine artery course circumferentially within the myometrium, sending radial arteries outward toward the endometrium and inward toward deeper muscle, creating a layered superficial pattern across the uterine fundus, body, and cervix. Along the lateral uterine margins, the main uterine arteries ascend in the broad ligament toward the cornu, where they anastomose with the ovarian arteries near the uterine tube. Inferiorly, cervical and vaginal branches descend toward the upper vagina, while venous channels parallel the arterial pathways and converge toward the uterine venous plexus. Orientation is anatomical: fundus superior, cervix inferior, uterine horns superolateral. This vascular map matters when you need to predict where bleeding originates and how it can be controlled. Ligation of the uterine artery at the level of the cervix during hysterectomy or cesarean hysterectomy relies on appreciating its close relationship to the ureter as it passes inferior to the artery (water under the bridge), and the collateral flow from ovarian artery anastomoses can sustain hemorrhage if not anticipated. Uterine artery embolization for fibroids and postpartum hemorrhage also targets this branching hierarchy, where hypertrophied arcuate and radial vessels feed leiomyomas and placental bed bleeding.
