Description
Prominent uterine arteries course medially within the base of the broad ligament toward the cervix, then ascend along the lateral uterine wall between myometrium and serosa, giving arcuate arteries that sweep circumferentially within the myometrium. From these arcuate vessels, radial arteries run perpendicularly toward the endometrium, branching into straight arteries supplying the basal layer and spiral arteries that extend superiorly into the functional layer. Venous channels mirror the arterial pattern as a uterine venous plexus draining laterally toward the internal iliac veins, with anastomotic connections to ovarian veins through utero-ovarian vessels near the uterine cornua. Layering matters. That surface-to-depth organization explains the endometrium’s rapid cyclic proliferation and its predictable bleeding patterns. Spiral arteries undergo hormonally driven remodeling and vasoconstriction during menstruation, and failure of normal placental bed spiral artery transformation underlies uteroplacental insufficiency and preeclampsia. During hysterectomy and uterine-sparing procedures, the uterine artery’s course adjacent to the ureter (the classic water under the bridge relationship) frames the safe plane for ligation near the cervix while preserving lateral pelvic structures.
