Description
Arising from the pisiform and the tendon of flexor carpi ulnaris, the abductor digiti minimi forms the most ulnar component of the hypothenar eminence, then runs distally along the medial margin of the palm toward the base of the proximal phalanx of digit V. From a posterior perspective, its muscle belly is seen lying superficial to the opponens digiti minimi and medial to the flexor digiti minimi brevis, with fibers tracking toward the ulnar side of the fifth metacarpal and the fifth metacarpophalangeal joint. Adjacent carpal and metacarpal landmarks, including the hamate and pisiform region, anchor the proximal attachment visually. Orientation is clear. Clinical relevance centers on ulnar-sided hand pain and intrinsic muscle weakness. Because the abductor digiti minimi is innervated by the deep branch of the ulnar nerve, wasting of the hypothenar eminence and loss of little-finger abduction can localize lesions distal to Guyon’s canal, and the posterior view helps you separate intrinsic atrophy from more proximal ulnar neuropathy patterns involving flexor carpi ulnaris or the ulnar half of flexor digitorum profundus. This angle also supports teaching of surgical corridors for ulnar nerve decompression at the wrist and for hypothenar flap planning, where preserving the ulnar neurovascular bundle and understanding the muscle’s superficial course matters. Hand anatomy courses and operative anatomy labs will use this plate to teach hypothenar compartment layering, tendon and bony landmarks around the pisohamate region, and functional testing of digit V abduction. It also fits well in textbooks and patient education materials addressing ulnar tunnel syndrome, intrinsic muscle denervation, or post-traumatic hypothenar pain syndromes.
