Description
Running along the medial thigh in an anterior male view, the adductor longus appears as a flattened triangular muscle superficial to the adductor brevis and lying medial to the pectineus and iliopsoas near the hip. Its proximal tendon originates from the body of the pubis just inferior to the pubic crest and pubic symphysis, then the muscle belly fans inferiorly and laterally to insert along the middle third of the linea aspera of the femur. Superiorly, the inguinal region and pubic rami frame the origin; distally, the muscle tapers as it approaches the femoral shaft. The gracilis tracks more medially as a strap-like companion. Clinically, this anterior perspective maps directly onto the groin pain patterns of adductor longus strain, common in cutting and kicking sports where rapid hip abduction and extension load the proximal tendon near the pubic attachment. Palpation and diagnostic ultrasound often target the proximal myotendinous junction, and this view helps clarify why pain can localize close to the pubic symphysis and mimic osteitis pubis or athletic pubalgia. The adductor longus also forms the medial boundary of the femoral triangle and contributes to the anterior wall of the adductor canal proximally, relevant when planning approaches around the femoral vessels.
