Description
Centered within the renal sinus, an ampullary renal pelvis forms a broad, funnel-like collecting cavity that receives urine from short, blunted major calyces with minimal branching. A cross sectional plane through the kidney typically places the renal cortex peripherally and the renal medulla deeper, with papillae projecting medially toward the pelvis rather than into elongated minor calyces. The pelvis continues inferiorly and medially into the proximal ureter at the pelviureteric junction, while segmental vessels and sinus fat occupy the remaining sinus space around the collecting system. Ampullary anatomy matters because it changes how the collecting system behaves under obstruction and how it is interpreted on imaging. On intravenous urography or CT urography, the wide, undifferentiated pyelum can mimic early hydronephrosis, and radiology trainees often overcall dilatation when they expect a dendritic pattern of calyces. Endourology also feels the difference: during ureteroscopy or retrograde pyelography, the roomy pelvis and shortened calyceal necks alter access angles to papillae and can influence planning for stone extraction or percutaneous nephrolithotomy. A normal variant. Easy to misread. Use this illustration in renal anatomy and urogenital embryology teaching to contrast ampullary versus dendritic collecting systems, and in radiology or urology materials that address pitfalls in diagnosing hydronephrosis or interpreting a renal pelvis in cross section.
