Description
Superior calvarial bone has been removed to expose the intracranial compartment, with the dura mater (pachymeninx) lining the internal table of the skull and closely investing the cerebral hemispheres. The dural layer lies superficial to the arachnoid mater and pia mater, and it forms a firm interface between bone and brain along the frontal, parietal, and occipital convexities. Anteriorly, the orbits and globes remain in situ, while the inferior cranial contents suggest the sellar and parasellar region where dura reflects to form diaphragma sellae and dural sleeves around cranial nerves. Color separation helps distinguish dura from cortical gyri and sulci. Dura matters in practice because it is the boundary that shapes common extra-axial hemorrhage patterns and dictates surgical planes. Separation of dura from the skull at the pterion and along the squamous temporal bone underlies epidural hematoma, classically after middle meningeal artery injury, while tearing of bridging veins between cortex and dural venous sinuses produces subdural hematoma that can track widely along the convexity. During craniotomy, the surgeon works in the epidural space to elevate and protect dura, then opens it with planned flaps to avoid the superior sagittal sinus and cortical draining veins.
