Description

Posteriorly oriented anatomy of the male lower leg and foot is rendered with the tibia positioned medially and the fibula laterally, framing the interosseous membrane and the distal tibiofibular syndesmosis at the ankle. Extensor hallucis longus is traced from its proximal attachment on the anterior surface of the fibula and adjacent interosseous membrane, its slender belly lying deep to tibialis anterior and extensor digitorum longus as it courses distally toward the dorsum of the foot. At the ankle, its tendon passes anterior to the tibiotalar joint beneath the superior and inferior extensor retinacula, then runs along the dorsal first ray to insert on the dorsal base of the distal phalanx of the hallux, medial to the extensor digitorum longus tendons. Extensor hallucis longus matters because it is the most specific motor for great toe extension and a reliable bedside marker of deep fibular (peroneal) nerve function, commonly referenced in the L5 myotome exam when assessing radiculopathy or peroneal neuropathy after fibular head injury. Its long tendon is also a frequent pain generator in anterior ankle and dorsal midfoot tenosynovitis, and it becomes a structure at risk during anterior ankle arthroscopy portals and dorsal approaches to the first metatarsophalangeal joint, where misidentification can lead to iatrogenic tendon injury and postoperative hallux extension weakness.
