Description

Running along the posterolateral compartment of the male leg, the fibular (peroneal) artery branches from the posterior tibial artery distal to the popliteal fossa and tracks inferiorly close to the medial aspect of the fibular shaft. Proximally, the popliteal artery and vein occupy the posterior knee region before dividing into the anterior tibial and posterior tibial systems, while paired venae comitantes accompany the deep arteries through the calf. More superficially, the great saphenous vein courses on the medial side of the leg toward the femoral vein, contrasting with the deeper venous channels that mirror the arterial paths into the ankle and foot. Orientation is clear. Red arterial conduits and blue venous return run in parallel from thigh to toes. Appreciating the fibular artery’s relationship to the fibula and interosseous membrane matters when planning lateral approaches to the distal leg and ankle, and when interpreting angiography for critical limb-threatening ischemia. Perforators from the fibular artery support common reconstructive options, including fibular osteocutaneous flaps and pedicled perforator flaps for distal third leg defects, where injury or spasm of this vessel can compromise both bone and soft-tissue perfusion.
