Description
Running along the anterolateral mandible, the depressor labii inferioris (inferior lip depressor) is shown as a quadrangular slip that arises from the oblique line of the mandible and ascends to blend with the orbicularis oris and the skin of the lower lip. Medial to it sits the mentalis, forming a paired complex at the chin where the depressor labii inferioris lies lateral and slightly inferior to the mentalis belly. A lateral left-sided view also places the muscle anterior to the masseter and inferior to the modiolus region, with the platysma and depressor anguli oris expected nearby in the superficial perioral plane. Fiber direction is clear. Depressor labii inferioris matters because it is one of the key elevators of asymmetry when the marginal mandibular branch of the facial nerve is injured, most often during submandibular gland surgery, neck dissection, or facelift procedures. Lower lip droop and an inability to evert or depress the ipsilateral labium inferius can be mistaken for primary orbicularis oris weakness, so isolating this muscle’s position relative to the mentalis helps clarify exam findings. The mandibular attachment also makes it a practical landmark when planning perioral incisions or discussing chin wounds and scar contracture.
