Running along the anterolateral mandible, the depressor labii inferioris lies deep to the beard-bearing dermis and superficial fascia, descending from the oblique line of the mandible to blend into the skin and mucosa of the lower lip. Its fibers sit medial to the depressor anguli oris and lateral to the midline chin region, with the mentalis occupying a more anterior and inferior position over the symphysis menti. The lateral composite also places the muscle in context with the mandibular body, lower dentition, and adjacent facial soft tissues. Orientation is clear. Functionally, the depressor labii inferioris pulls the lower lip inferiorly and slightly laterally, shaping expressions such as pouting and exposing the mandibular incisors. That action matters in facial nerve anatomy: the marginal mandibular branch of the facial nerve courses along the inferior border of the mandible and is vulnerable during submandibular incisions, facelift (rhytidectomy) dissection, and repair of mandibular fractures. Weakness of this muscle contributes to lower lip asymmetry and drooling in peripheral facial palsy, and the muscle is a common target, directly or indirectly, in chemodenervation planning for perioral dystonia.
