Description
Positioned deep in the lower lateral neck, the jugular trunk is rendered as a green lymphatic channel ascending along the internal jugular vein and converging near its termination at the venous angle, where the internal jugular vein meets the subclavian vein. Superficial and deep cervical lymph nodes cluster along the anterior and posterior margins of the sternocleidomastoid muscle, with efferent vessels coursing medially toward the jugular chain. Superiorly, lymphatic pathways from the submandibular region and along the mandibular ramus feed into the deep cervical nodes, while adjacent blue nerve pathways thread between muscle bellies and fascial planes. Mandible and skull base landmarks anchor the relationships, and the cervical musculature is shown in fiber-level detail. Attention to the jugular trunk matters because it is the final common conduit for deep cervical lymph drainage, and its short course near the venous angle places it directly in the operative field during internal jugular vein cannulation, neck dissection, and central line placement. Injury or ligation can contribute to postoperative lymph leak, and understanding laterality helps, the right jugular trunk typically joins the right lymphatic duct, while the left often enters the thoracic duct at the left venous angle. Small structure, real consequences. For teaching, the juxtaposition of lymphatics with sternocleidomastoid, carotid sheath contents, and nearby cranial nerve trajectories clarifies why metastatic spread from oral cavity, pharynx, and larynx targets the deep cervical chain before reaching terminal lymphatic ducts.
