Description
Anterior surface anatomy places the pediatric liver predominantly in the right upper quadrant, deep to the costal margin and extending medially into the epigastrium. A semi-transparent hepatic silhouette occupies the right hypochondriac region, with the inferior margin approaching the right costal arch and the left lobe tapering toward the midline. Inferior and slightly medial to the right lobe, the gallbladder sits on the visceral surface, a small landmark that helps orient the hepatic contour. Proportions matter. In a female child, the liver appears relatively large for body size, and its inferior edge may be palpable below the right costal margin without implying hepatomegaly. That distinction becomes clinically relevant when teaching pediatric abdominal examination or correlating a physical finding with common differentials such as viral hepatitis, congestive hepatomegaly from right-sided heart failure, biliary obstruction, or storage disorders. The anterior projection also supports interpretation of hepatobiliary ultrasound planning, where rib shadowing and the subcostal approach influence how you angle the probe to assess parenchyma and the gallbladder.
