Description
Positioned in the right upper quadrant, the pediatric liver occupies the subcostal space beneath the right hemidiaphragm and deep to the lower ribs. From an anterior perspective, the right lobe dominates laterally while the left lobe crosses the midline into the epigastrium, approaching the left hypochondrium. The inferior margin follows the costal arch, and the superior surface conforms to the diaphragm, placing hepatic parenchyma (hepatocyte-rich tissue) immediately subjacent to the anterior abdominal wall in a lean child. Surface anatomy is the point. Clinical correlation matters because liver size and position in children differ from adults, and a normally palpable inferior edge may reflect age and body habitus rather than hepatomegaly. This orientation also frames common injury patterns: the right lobe is more exposed in blunt abdominal trauma, and its proximity to the ribs and diaphragm explains referred pain to the right shoulder via the phrenic nerve when the capsule is irritated. For procedures, anterior surface landmarks guide safe subcostal access for ultrasound assessment, biopsy planning, or drainage, while avoiding pleura and lung bases that can descend with inspiration.
