Description
Shown in lateral profile, an obese adult white male torso is rendered semi-transparently to localize the liver in situ beneath the right hemidiaphragm. The right lobe occupies the right upper quadrant, projecting inferior to the costal margin and lying lateral to the stomach, while the left lobe extends medially toward the epigastrium. Sketching of the stomach and small and large intestines provides abdominal context, with the hepatic parenchyma suggested at a finer scale as lobules within the organ outline. Obesity changes the clinical conversation around hepatic anatomy. A larger, more inferiorly projecting liver is a common external correlate of hepatomegaly, and in an obese patient the differential often centers on metabolic dysfunction associated steatotic liver disease (formerly NAFLD) rather than alcoholic liver disease. This side view also helps teach why right upper quadrant discomfort can be nonspecific, and why palpation and percussion of the liver edge can be unreliable when the abdominal wall is thick and the diaphragm position shifts with body habitus. Surface anatomy matters.
