Description
Running through the medial quadrants of the female mammary gland, green lymphatic vessels course from the glandular lobules and lactiferous duct system toward the parasternal region, passing through the subareolar plexus and along fibrous septa within the adipose stroma. Deep to the breast parenchyma, the pectoralis major forms the posterior boundary, while superficial connective tissue (including Cooper ligaments) partitions fat and gland as the vessels converge medially. Lobules sit more posterior and peripheral, ducts track anteriorly toward the nipple and areola. Orientation is clear. Medial lymphatic drainage matters because it is the pathway most likely to cross the midline and reach internal mammary (parasternal) lymph nodes, a route that can be underappreciated when attention focuses only on axillary nodes. In breast carcinoma of the inner quadrants, metastasis to the internal mammary chain influences staging and radiotherapy field design, and it also explains why a clinically negative axilla does not always exclude nodal spread. Surgeons and radiologists use this anatomy when correlating sentinel node mapping with SPECT-CT, planning parasternal node sampling, or interpreting medial breast edema and skin thickening patterns.
