Description
Medial lymphatics of the female mammary gland are traced from the glandular lobules through the branching duct system toward the areola and nipple, with lymphatic channels running within the connective tissue septa between adipose lobules. Deeper planes relate posteriorly to the pectoralis major fascia, while superficial lymphatics course anteriorly in the subcutaneous tissue. Medially, the dominant collecting pathways converge toward the parasternal (internal thoracic) lymph nodes along the sternum, a key route alongside the better-known axillary drainage. Medial breast lymphatic drainage matters because it explains patterns of spread that can be missed if you think only in terms of the axilla. Tumors in the medial quadrants more often drain to the internal mammary chain, a pathway associated with involvement of parasternal nodes and potential cross-midline drainage to the contralateral breast. This is the anatomy behind internal mammary sentinel node mapping, and it underpins radiotherapy field design when parasternal nodal disease is suspected or confirmed. A common blind spot.
