Description
Running superficially from the intercostal space toward the breast parenchyma, the anterior branch of the lateral cutaneous branch of an intercostal nerve is traced as it pierces the intercostal musculature and serratus anterior near the midaxillary line, then courses anteromedially within the subcutaneous tissue. Mammary lobules and adipose tissue occupy the anterior chest wall, with lactiferous ducts converging toward the nipple-areolar complex. Deep to the gland, pectoralis major forms the muscular bed, separated from the posterior breast by the retromammary space, while small cutaneous nerve twigs approach the skin from a superficial plane. This nerve pathway matters when you need to explain breast and anterolateral chest wall sensation in real clinical terms: the lateral cutaneous branches (and their anterior branches) account for much of the cutaneous innervation to the lateral breast and are frequent sources of postoperative neuropathic pain after mastectomy, lumpectomy, or axillary dissection. During lateral approaches, the point where the nerve emerges through serratus anterior and the superficial fascia is a predictable site of traction or transection. Small nerve. Big symptoms.
