Description
Centered on the female mammary gland, the anterior branch of the lateral cutaneous branch of an intercostal nerve courses from the lateral thoracic wall toward the nipple areola complex, passing through the superficial fascia overlying pectoralis major. Lobules of glandular tissue sit anterior to the pectoral fascia, with lactiferous ducts converging medially toward the nipple while the cutaneous nerve branches ramify in the subcutaneous plane. Blue vascular and lymphatic channels track alongside the glandular parenchyma, with fine neural twigs running between lobules and toward the overlying skin. Orientation is anterior, with lateral structures leading into more medial ductal convergence. Cutaneous innervation of the breast is not just an academic detail, it is where patients feel surgery. The lateral cutaneous branches of the intercostal nerves, with an anterior branch supplying the anterolateral breast skin, are commonly encountered during lateral dissection in mastectomy, oncoplastic reduction, and axillary tail mobilization, and injury can produce postoperative hypoesthesia, dysesthesia, or chronic neuropathic pain along the anterolateral chest. This view also supports teaching of referred pain patterns from thoracic dermatomes and clarifies why incisions placed laterally can disturb sensation even when the nipple itself is preserved.
