Description
Arising from an intercostal nerve in the anterior thoracic wall, the anterior cutaneous branch courses medially toward the parasternal region, then pierces the intercostal musculature and pectoral fascia to reach the subcutaneous plane overlying the breast. Superficial to pectoralis major, it divides into medial and lateral cutaneous twigs that run toward the skin of the anterior chest and the areola, tracking between lobules of glandular tissue and the surrounding adipose. Nipple-areolar skin sits most anteriorly, with lactiferous ducts converging centrally, while venous or lymphatic channels accompany the neurovascular network through the superficial fascia. Sensory innervation to the breast is clinically concrete, and the anterior cutaneous branches of the intercostal nerves are frequent culprits in postoperative numbness or dysesthesia after procedures that traverse the parasternal and periareolar skin, including lumpectomy, mastectomy incisions, and implant pocket dissection.
